Post-bariatric body contouring (PBBC) refers to a group of surgical procedures that remove excess skin and fat in an effort to improve appearance and function for a patient who has lost weight with or without the help of bariatric surgery.
The decision to pursue body contouring is very personal. It’s not expected that all weight loss patients will benefit from or desire this service. It’s also quite difficult to predict which massive weight loss patients are likely to choose to have body contouring surgery.
BENEFITS OF POST BARIATRIC SURGERY
Patients who do choose to have PBBC are often motivated to do so for a combination of reasons. The most commonly treated area is the abdominal region. Excess skin in this area of the body can restrict physical activity, lead to self-conscious behaviour and remind an individual about his or her prior weight. The right body contouring procedure would address all of these issues.
The most common body contouring procedures for massive weight loss patients are:
Tummy tuck (abdominoplasty)
Breast lift (mastopexy)
Brachioplasty
A brachioplasty, or arm lift, is a powerful procedure that removes excess skin from the upper arms, axilla and lateral chest. This often results in a dramatic improvement in the contour of the upper arm and elimination of the so called ‘bat-wing’ deformity. The scar is concealed on the inner surface of the arm. For patients with stubborn fat deposits in this area, liposuction can be included as part of the procedure.
Upper Body Lift
An upper body lift targets excess skin and fat in the lateral chest and upper back region. This procedure can be effectively and safely combined with a brachioplasty or a breast lift. The resulting scar is concealed in the location of the “bra strap”.

Lower body lift
A lower body lift is often considered to be the cornerstone of post-weight loss body contouring. Most weight loss patients identify the abdomen as their primary concern and this procedure is often selected as the initial step in body contouring. A lower body lift includes a full abdominoplasty (tummy tuck) and in addition lifts the lateral thighs and buttocks. Scar position is carefully planned and is typically kept low.

Medial Thigh Lift
Fluctuations in weight often leave the medial thighs with persistent skin laxity and stubborn fat deposits located both superiorly and down near the knees. A medial thigh lift effectively addresses these issues with a combination of liposuction and surgical excision. The resulting scar is concealed along the inner aspect of the thigh.

Fat Grafting
Massive weight loss patients often have stubborn fat deposits, but it’s also not uncommon to have localized areas that appear relatively deflated. Lifting procedures do not usually correct the volume loss in these areas and fat grafting is often helpful to re-volumize them and restore a youthful, natural and proportional appearance. Fat grafting is a minimally invasive procedure that allows us to transfer your own fat from an area of relative excess to an area of relative deficiency. Fat is collected using a liposuction technique, carefully prepared for transfer and then injected through tiny incisions in to the desired areas such as the face, breasts, lateral hips and buttocks. Fat grafting is often combined with other surgical procedures.
Facelift
Massive weight loss can accelerate age-related changes of the face and neck. The most noticeable changes include jowl formation, accentuation of the nasolabial folds and loosening of the neck. A lower facelift repositions descended cheek and neck tissues allowing re-creation of a well-defined and youthful jawline and neck. Excess skin is removed while taking care to avoid any unnatural tightness. The scars are very carefully concealed around the ear and along the hairline. In addition, there is volume loss around the eyes and in the midface that contributes to an age-related change in the projected emotional expression of your face at rest. Micro fat grafting, in combination with a facelift, helps to restore the fullness of a youthful appearance.
HOW DOES POST BARIATRIC SURGERY WORK?
Making the decision about when to have body contouring surgery is an important one. These procedures are often major life events that require time off work, careful planning and help from friends and family. Your weight should be stable for at least 6 months prior to surgery. A healthy diet and an active lifestyle are also beneficial.
A proven process helps to ensure the best possible result. Each of these steps has been developed or adapted specifically to meet the needs of massive weight loss patients:
-
- Patient education
- Development of an individualized surgical plan
- Pre-operative medical work-up
- The operation(s)
- Standardized post-operative care
- Assessment of the final outcome
Consultations are only available through the Specialist Referral Clinic and all surgical procedures take place at the Cambie Surgery Centre. Patients will typically meet with Dr. Reid once prior to surgery. If needed, however, patients are welcome to arrange a second visit, or a phone call, to discuss any changes in their health, the results of pre-operative investigations or any questions that may arise.
The consultations are thorough and informative. Expect to:
-
- Review your weight loss history and your goals for body contouring
- Review your past medical history
- Have a focused physical examination
- Receive a personalized surgical plan
- Discuss the expected outcome, the typical recovery and the potential risks
- Receive a quote for private surgery

- Begin the planning for your recovery phase
